I agree to hold Allie’s Pet Grooming NYC Inc harmless from damage, loss or claims arising from any condition of my dog(s) whether known or unknown. These damages, loss or claims shall include but are not limited to death and injury. Pre-existing conditions shall include but are not limited to illness, previous injury, skin and coat conditions, medical conditions, advanced age and nervousness.
In the event of an emergency, I authorize Allie’s Pet Grooming NYC Inc to act as an agent for any veterinary treatment for the cat as necessary. I agree to pay for all charges that may incur as a result.
Dematting/detangling of matted hair may cause patchiness and hair loss. Consent to remove the matted hair is on this understanding. Shaving your dog will dramatically change the pet’s appearance, and the hair will be very close to the skin. Severe matting and/or pelting will result in an additional surcharge, based upon the severity of the problem and the time required to correct it. A fecal removal surcharge and/or biter surcharge may also apply.
Allie’s Pet Grooming NYC Inc reserves the right to refuse any service at any time.
I give my permission to Allie’s Pet Grooming NYC Inc to take photos and videos of my dog(s) to use for business purposes.
All payment is due at the time the services are rendered.
New clients: I agree to send a non-refundable non-transferrable $40 deposit to hold the spot.
Existing clients: Cancelation 48 hours before the appointment results in a $40 fee. Repeat offenders (frequent rescheduling regardless of timing) will have to send a non-refundable non-transferrable $40 deposit to hold a spot for all future appointments.
